Note - New Student-Athletes beginning the Spring Semester (Jan 8th) of 2024 will need to present the following to the Head Coach or General Manger on the first day (Jan 8th):

· Proof of Medical Insurance - provide a copy of your Medical Insurance Card (front & back).

· Medical Clearance – complete the Sports Physical Form – print the physical form and complete as necessary.

· Fitness Declaration, Waiver and Release of Liability. Click on the following link to access, download and complete this Waiver – Note - if you are under 18 years of age you must also have a parent’s signature.
