Reporting Date checklist

Equipment –

Most All equipment will be provided. Players will need to provide:

· Practice Socks.

· Under T-shirt for shoulder pads.

· Cleats.

· Any Braces that may be desired.

Medical Clearance –

· Provide a copy of a valid Athletic Physical indicating that you are cleared to participate in contact football. The Physical should have been completed within two-months before reporting date. If you need a blank physical form to provide to your Doctor you can click and download ours-- Sports Physical Form

-You will need to bring the completed Physical form with you to check-in. We must have a Physical before you will be allowed to participate.

· Proof of Medical Insurance - Provide a copy of your Medical Insurance Card (front & back). This can be uploaded into Airtable. Medical Insurance Link

· Fitness Declaration, Waiver and Release of Liability.  Click on the following link to access, and complete this Waiver – Note - if you are under 18 years of age you must also have a parents signature.

· Braces – if any braces are desired/needed, Athletes will be required to provide their own (i.e., ankle, knee & shoulder … etc).

·NOTE –again, not having the documents listed above properly filled out will delay participation.

Communicate with your Coach and our Academic Office –

· You should be in regular communication with your Coach and our Academic Support to make certain you are prepared for Fall Camp, Academic's and the Season.

USA-C Support Fee -

· This fee is due before reporting to Camp.  If you are having problems coming up with the support fee please contact your coach as he can help you with options.
