DATE : SATURDAY, MARCH 16TH, 2024
TIME : 1PM- 4PM
LOCATION : DIXIE HIGH SCHOOL
PRICING: ONLINE PRE-REGISTRATION: $50
DAY OF REGISTRATION: $75
Athlete MUST Upload*:
Proof of current medical Insurance (copy of insurance card). Insurance should be under your parents, yourself, or your spouse through another plan.
Provide a copy of a valid current Athletic Physical, or Doctor Dictation from the visit. Unless a significant injury, a Physical Form for the 2023 Football Season should suffice.
Signed copy of the Fitness Declaration, Waiver and Release of Liability (if under 18 must also be signed by parents).
Athlete is responsible for their own work-out gear,
including cleats.
